Registration Form 2008-2009

  Age - Pre-Kindergarten 3 through First Grade, must be the prescribed age before September 1 of the school year for which they are applying and must provide a birth certificate.

Tuition

 

  Academics - Grades second through eighth, must have successfully completed the previous year and must provide school records, achievement test scores, social security number, birth, and baptismal certificates.

Dress Code 

 

  Disciplinary - All students entering Our Lady of Mount Carmel from another school, public, or private, must have satisfactory grades in conduct.

The average cost  to educate a student here

at Our Lady of Mt. Carmel is $6,681

 

            

This cost is met through the following means:

 1.  Tuition                              Yearly              10 months    12 months

  PreK 3 - PreK 4 (full day)     $4,000                 $400              $333.33

  K - 5th grade                          $3,800                 $380              $316.66

  6th- 8th grade                        $4,100                 $410               $341.66

 

 2.  Fees

    Registration Fee - Returning $150, New $200

    Curriculum Fee - $400 per Student

 

 

Fees are due by August 8,2008

 3. Family Fundraising Commitment

 (Per Family/Per School Year)

 Single Child: $600

2+ Children: $750

           

 

Family Discount:

First Child:  highest of tuitions 

Additional child: $3,000 plus fees

 

 Additional Tuition Assistance may be available through PSAS

 
 

 

 

Refund Policy:

Fees are non-refundable. (Registration & Curriculum)

First month’s tuition is non-refundable.

Tuition is due for any month in which a student has attended classes. For example, a withdrawal on October 2nd will require that the October tuition be paid in full.

The refund policy applies to payments made monthly, semi-annually or annually.  In case of withdrawal, prepaid tuition will be refunded except for the above.

 

 

 
 

Payment Program:

Our Lady of Mount Carmel School adopted the tuition collection program called SMART.  Tuition payments are made through the SMART Program by automatic deduction or by check.  Payments may be made monthly (June - May) or (August –May), quarterly (June, September, January, April) or semi-annually (June & December).

 

 
 

Parental/Family Involvement:

All students enrolled at Our Lady of Mount Carmel  School receive tuition assistance to meet the educational cost per student which on average is $2,000 more than tuition.  Therefore, if you enroll your child/children at OLMC, you will be required to support the fundraising efforts of the school. We also expect parents/family members to share in the organizing and preparation of the different events of the school, through volunteer hours. .

      Other costs include: uniforms, extracurricular activities, school pictures, field trips, cheerleading,

      athletics, yearbook and lunch fees.

      Athletic fees are $85 per sport.

MISCELLANEOUS COSTS:

Special supplies are sold prior to the first day of school.

Other costs during the year include:  dress code clothing, extracurricular activities, enrichment programs, school pictures, field trips,  cheerleading, athletics, yearbook and lunch fees.

Dress Code

 
     
     All students must wear the official school uniform purchased from Parker School Uniforms.  Any student not in proper uniform will be sent to the school office to call a parent for a change before returning to class. 

     Parker Uniforms is located at 5430 Buffalo Speedway (713-661-6110).  Uniforms may also be purchased online at  www.parkersu.com .  Please check with Parker for proper uniform dress.  Elementary dress differs from Junior High dress. 

     Uniforms that must be purchased from the school office are as follows:

         P.E. T-Shirt & Blue Shorts        For Grades 5th thru 8th (required to change for  P.E.)  

         Red OLMC sweatshirts              For All Grades (K-3 - 8th)  

         OLMC Brown Sports Jacket       Grades 5th through 8th

 

   

     

 

Students may wear tennis shoes or casual shoes that are appropriate for school & P.E. activities. Socks for all students must be 2" above the shoe.  Shoes must be closed on both ends.  No Sandals or Flip-Flops.  Non-Skid Mark. No high heels or platform shoes.

       Students in grades 1st through 8th must wear a belt.

       All Students may wear a necklace with a religious medal.  Girls may wear stud earrings with a limit of 2 on each ear but not on cartilage.  Clear nail polish is acceptable.  Make-up and tattoos, facial/body piercing is prohibited.
